What is the UTICA Card? |

The UTICA Card is a non-insurance discount card that provides alternative health care solutions for active members, offering non-insurance answers, with significant savings for prescription drugs, dental services, vision care, chiropractic services, diabetes supplies and durable medical equipment, hearing services and much more. |

Who can use the card? |

The membership card can be used by the member and all other dependents living in his/her household who is financially dependent upon the cardholder. |

On most products and services, the discounts range from 10% to 60%. |

How do the benefits work? |
| A: |
You may begin using your benefits as soon as you receive your card. The membership kit comes complete with an explanation of benefits and a list of major chain pharmacies that participate in the program. For additional providers, you can call the toll free phone number on your membership card to locate participating providers in your area. You then take the card to the provider, select the desired product or service, and present the card at the point of sale. You will receive the contracted discount price and you must pay at the time service is rendered. |

When can I begin using my benefits? |

You can use your card on the day you receive your card. All you have to do is present the card when you arrive at the provider's location. |

How many times may I use the benefits? |
| A: |
Unlike insurance, utilization is encouraged because it directs traffic to participating provider locations. You and your family may use the benefits as many times as needed throughout the membership year. There are no limitations on usage. |

If a particular location is not on the provider list, how do I go about getting my practitioner or pharmacy on the panel? |

You may call the customer service number on the back of your card, or send an email to help@uticadirect.com with the name, address, and telephone number for the provider of their choice. Our customer service team will contact the provider's office and invite him/her to join the appropriate network. |

How do I know which providers in my area will honor the UTICA Card? |

To locate a participating provider in your area, use our On-line Provider Locator, or call the toll free number on the back of your membership card, Monday through Friday 9:00 AM to 6:00 PM EST and one of our friendly customer service representatives will be happy to assist you. Also, when you enroll you will receive an Information Package that will include a listing of the most commonly used healthcare providers in your area who are in the program. |

Is the UTICA Card an insurance program? |

No. This is not an insurance program. It is simply a way to obtain substantial savings on all of your healthcare needs. |

Can the benefits be used with insurance? |
| A: |
The plan can be used with some dental insurance plans only. If you already have insurance (i.e. a traditional 80/20 indemnity plan), you may use the benefits as follows: You pay the discounted amount in full to the dentist at the time services are rendered. Then you may file the claim yourself with your dental insurance company. This may also be beneficial to help you reach your deductible or lower overall out-of-pocket expenses.
Additionally, many insured dental plans have an annual expenditure cap where the insured pays 100% of all costs exceeding the cap. Using the discount dental plan in conjunction with insurance helps you to delay reaching the capped amount. |

Is there a deductible? |

No. This program is not insurance. As such, there are no deductibles, no waiting periods, no pre-existing condition exclusions, reimbursements, etc. Everyone qualifies and a physical is not required to purchase the card. |

Do I have to file a claim form to get the savings? |
| A: |
No. There are no claim forms or other paperwork to file. You receive your discount at the time of service and you must pay the provider the discounted amount in full at the time services are rendered. |

Insurance companies exclude people for pre-existing conditions to contain costs. How can the UTICA Card program allow everyone to qualify? |

Unlike insurance, discount programs do not require personnel for claims adjudication, nor is there a necessity for cash reserves because there is no risk. In exchange for giving a discount, providers join the network so that we will direct traffic to their business. |

What is the money back guarantee? |

If you are not completely satisfied with any of the benefits within 30 days of receiving your card, you may return the card for a complete refund. |
